To schedule a patient at a Mission Health System facility:

All orders must contain the following information on a single page:
• Patient’s Name
• DOB
• Date the order was written
• Name of ordering physician and/or other authorized individual
• Specific test to be performed (Modified Barium Swallow Study)
• ICD 10 Code
