If you, as the patient, are sending someone to pick up medical records on your behalf, you must indicate that person’s name on the HIPAA form where required and add a phone number where you, the patient, can be contacted.
Cost
There is no charge for the first set of films or CDs. If a second set is requested, a nominal fee will be charged.
